Taking care of you and your family is our highest priority. That is why when it comes to talking about finances, our goal is to provide you with clear information regarding our fees and your payment options.
Fees
First visit costs depend on the specific types of services required. Each new patient needs to have at least a basic exam so we can evaluate the current dental and oral conditions. If more extensive testing is desired or needed, or if we need to take x-rays, additional fees will apply. These will be discussed with you before setting your first appointment.
Because you are an individual with unique health conditions stemming from your unique dental and medical history, it is impossible to quote exact costs for any specific course of treatment before seeing you in our office. Your case will differ from the next person’s. Your priorities also will be different, as will the level of investment – financial, physical, emotional and so on – you are able and willing to make. So once Dr. Erwin has gathered and analyzed all the clinical information about your case, he will share his evaluations and recommendations with you. He will listen to your thoughts, feelings and desires, and answer any questions you may have. At this point, you will receive a treatment plan and quote for the work to be done on your journey to dental wellness.
Fees for any services provided by outside clinics or labs (e.g. blood tests, serum compatibility testing, panorex imaging) will be charged to the patient by the clinics or labs used.

Payments
Payment is expected in full at the time of service unless other arrangements have been made. We accept Visa, MasterCard, Discover and American Express. Or you can set up a monthly payment plan via CareCredit or Capital One Healthcare Finance. You can apply for either program online or ask our office for an application.
Insurance
We will gladly submit the paperwork for your insurance, making up to two attempts to obtain payment. When you visit our office, please make sure that we have all the necessary information, including up-to-date ID cards.
Though we will strive to see that you are reimbursed for covered benefits, please keep in mind that your benefits contract is between you, your insurance company and, as applicable, your employer. Also remember that dental plans are not designed to cover all procedures. We urge you to understand your specific benefits, including co-pay amounts, fee schedules and maximums per benefit year. Neither we nor your insurance company can guarantee any payments until your insurance company reviews your claim.
All fees for any treatment received by Dr. Erwin are the responsibility of the patient.

Dr. Erwin is a Delta Dental Premier Provider. All other Delta plans are considered out-of-network. Please understand your policy, as you will be asked for your co-pay at the time of service. Note that many of the
services we provide will NOT be covered benefits.
